Michael P. Kane, MD 

Board Certified Family Practice

Michael was born in Elmira, NY. He graduated from Canisius College with a degree in Biochemistry. He then attended medical school at the University at Buffalo Family Medicine in the Medalist Honors Program. Michael also completed his residency at UB in the department of Family Medicine. He is board certified with the American Board of Family Medicine.

Maria Matala-Sullivan, DO

Board Certified Family Practice

Born in Buffalo, NY, Maria earned her B.A. in Biology at Canisius College. She then attended medical school at the Philadelphia College of Osteopathic Medicine. Maria completed her residency at St. Margaret’s Hospital through the University at Pittsburgh in the department of Family Medicine. She has been a proud healthcare provider at Lakeshore Primary Care for more than 25 years.

Maria was recognized by her peers as an expert in Family Medicine for Buffalo Spree Magazine's Top Doctors of 2024 list. Maria is also a Diplomat of the American Academy of Family Practice and enjoys traveling, sporting events, skiing, and spending time with her family.

Jessica E. Ogorchock-Congilosi, MD

Board Certified Family Practice | CMP Board Member

Jessica was born in Niagara Falls, NY and first graduated from Le Moyne College with her B.S. in Biology. She attended medical school at the University at Buffalo and completed her residency there in the department of Family Medicine.

Jessica lives in Hamburg, NY with her husband and two sons. Her interests include spending time with family, cooking, traveling.

Dennis M. McGoldrick, PA/C

Dennis was born in Buffalo, NY and graduated from the University at Buffalo School of Pharmaceutical Studies. From there, he graduated from the Physician Associate Program at the Yale University School of Medicine. He then went on to complete his residency at the Buffalo General Clinical Pharmacy.

Dennis lives in the Hamburg, NY area. He has been with Lakeshore Primary Care Associates for more than 30 years. He enjoys spending time with his family, playing hockey, and going on long walks. Dennis is a former English literature major continues to be an avid reader.

Sara Burgio, FNP-C

ANCC Board Certified Family Nurse Practitioner

Born in Buffalo, NY, Sara earned her associate degree in nursing from Erie Community College and then went on to earn her bachelor’s degree in nursing from Daemen College. She then earned her master’s degree in nursing – Family Nurse Practitioner from Duke University.

Sara has been a nurse practitioner since 1998. She joined the team at Lakeshore Primary Care Associates in the fall of 2002. She has a strong clinical background in women’s health and pediatrics. Sara enjoys spending time with her patients with a focus on patient education to empower them in life transitions and assist with health maintenance and disease prevention and management.

Sara lives in Eden, NY with her husband, identical twin daughters, and three sons. Her interests include spending time with family, reading, and outdoor activities.

Janele Storms, FNP-BC  

Janele was born and raised in Buffalo. She graduated from Trocaire College in 2016 with an associate degree in nursing, becoming a Registered Nurse. In 2017, while completing her Bachelor of Science in nursing at Daemen College she worked as a bedside nurse in acute care. Janele then went on to obtain her Master of Science in nursing to earn the title of Family Nurse Practitioner at D'Youville University in 2019.

Janele has been working at Lakeshore Primary Care Associates as a NP for 4 years. When she isn't working Janele enjoys reading, running and spending time outdoors.
